The polyptych presents the Madonna and Child at the centre, flanked by saints beloved to the Franciscan order. To the left of the Virgin, in a prominent position, stands Saint Francis of Assisi, the titular saint of the convent; to her right Saint Anthony of Padua. The lateral panels are occupied by figures of saints connected with the L’Aquila area — Bernardino of Siena and John of Capestrano, founder of the monastic community. John is the only figure shown with a radiant halo, as he was venerated as Blessed even before his official beatification, which took place in 1650.
The cusps above show, at the centre, the figure of Christ in pietà accompanied by the instruments of the Passion. In the sides the Saints John the Baptist, Jerome, Bonaventure of Bagnoregio and Louis of Toulouse are depicted.
The predella hosts, at its centre, Christ blessing, surrounded by the company of the apostles. Saint Clare of Assisi and Saint Elizabeth of Hungary stand at the two ends.
